Type 2 Diabetes and the Young – The T2Day programme
If you are aged between 18 and 39 years old and have been diagnosed with type 2 diabetes, you will soon be invited for an enhanced review to help you manage your condition.
Being diagnosed with type 2 diabetes earlier in life is associated with higher lifetime risk of health problems than when diabetes develops in older age. As well as supporting you to reduce these risks, this review will help you to:
- Control your blood sugar levels
- Reduce your future risk of having a heart attack or stroke
- Manage your weight
- Look after your psychological and emotional wellbeing
- Plan for healthy pregnancy
- Access other local services to help you live well with type 2 diabetes
It’s really important to take up this support when offered
National Diabetes Audit
The surgery is taking part in a national project called the National Diabetes Audit (NDA).
The project is about people at risk of diabetes and diabetes care and treatment in the NHS.
