Forms can be emailed or brought into the practice for the nurse to assess.

- Please allow 5 working days for a response before contacting the practice regarding your vaccinations.
- Once the assessment is completed you will then be able to book a consultation with the nurse.
- Please note that you should be checking for any required vaccinations 4 to 6 weeks prior to travel, therefore we kindly ask you to give plenty of notice when submitting a Pre-Travel Assessment.
- Please ensure you book your consultation well in advance of travel, as pressures may result in us not being able to carry out the vaccinations at the surgery if booking at short notice.
