To use you will need a Practice ID number and Access ID number which is obtained from the practice, and, in conjunction with a password, will be unique to you.

Please fill out our Online Access Form (for age 16 and over) and send this back to us on our email - REDCCG.NewburyGroupPractice@nhs.net

You also need to include a copy of your Photo ID.

PROXY online access form to be used for:

• A patient who is aged under 11
• An adult who lacks capacity (we will use the medical records to determine if proxy access appropriate)
